MaST Schools Volunteer Application

Welcome to MaST's new application website for prospective volunteers!  

We appreciate your interest in volunteering at MaST schools. Regardless of which MaST school you are looking to volunteer at, this is the application that must be completed. Please note that you are NOT an approved volunteer until you receive an email indicating that your application is approved.  Furthermore, staff members are instructed not to bring in volunteers unless they are on the approved volunteer list.

It is very important that the information you enter on the application matches your government-issued identification card including your full legal name.  It is important that you provide a valid email address so you can be notified as to the status of your application and for future communication.  All information collected on the application will remain confidential and not be shared outside the volunteer program.

Before completing this application, please note that 4 clearances must be obtained as part of the application.  When completing this application, please ensure you have all 4 clearances before completing and upload at the appropriate spot.  If you do not know how to upload documents and/or are unable to attach the requirements, you can bring the 4 clearances to the front desk at the school you want to volunteer at and somebody can help you complete this online application.  Directions for obtaining these clearances, FAQs, and the application process can all be found at our Volunteer Clearances website.  Please review this website carefully as it explains all important information as it relates to volunteer clearances.  Please copy and paste this link into a separate browser/tab so that this page remains open:

Finally, please note that if/when you become an approved volunteer and then are convicted of an offense that would constitute grounds for denying further participation as a school volunteer, you must provide MaST Schools with written notice not later than 72 hours after the arrest, conviction, or notification that you have been listed as a perpetrator of child abuse in the statewide database.

IMPORTANT:  MaST Schools needs EITHER a MaST Schools Volunteer Affidavit OR the UEID number you receive when registering if doing the FBI Fingerprinting to fulfill this one clearance requirement.  In the appropriate attachment field below, please attach whichever of those two you are completing for that clearance requirement.  As a reminder, if submitting a UEID number, it must be as an attachment (not a text field).  For all of these attachments, they must be in one of the following formats: PDF, JPG, PNG.  We will accept whatever method works best for you (including pictures from a phone) as long as we can very clearly read all of the text in the entire document.
