We appreciate your interest in volunteering for the District Volunteer Program. Our district offers a variety of activities you may choose from when volunteering your time. Simply complete and submit a volunteer application and upon approval you will receive instructions on next steps.It is very important that the information you enter on the application matches your government issued identification card including your full legal name. It is important that you provide a valid email address so you can be notified as to the status of your application and for future communication. If you are unable to provide an email address, you use the district’s email address. All information collected on the application will remain confidential and not be shared outside the volunteer program.
PLEASE READ: If you select the function of Coach, Overnight Chaperone, or Mentor, you have to be FINGERPRINTED. If you select any of these three options you will have to email email@example.com to schedule that process. Selecting any of these will slow down your approval process for all areas of volunteering.

I am aware that for the protection of students, the Okaloosa County School District requires and conducts FDLE Sexual Offender and Predator screenings on all volunteers. National and Florida Department of Law Enforcement background checks are conducted on all school mentors and spot background checks on school volunteers. In addition, volunteer coaches and overnight field trip chaperones will be fingerprinted. I hereby approve of such a check on my background.
I am also aware that Social Security Numbers shall be collected from District volunteer program applicants for:
(a) Initial background screening not requiring fingerprints for any volunteer applicant who declares, or has had, a prior arrest; and,
(b) For full criminal background screening (Level II Screening- §435.04, Fla. Stat.) for service as a one-on-one mentor, over night field trip chaperone, volunteer coach, or random applicant volunteer screening.

For the protection of our students, the Okaloosa County School District requests a response from each school volunteer to the items in this form. Volunteers will be asked to complete a new Volunteer/ Mentor Affidavit each school year.
