District Volunteer Application

We appreciate your interest in volunteering for the District Volunteer Program. Our district offers a variety of activities you may choose from when volunteering your time. Simply complete and submit a volunteer application and upon approval you will receive instructions on next steps.  Please discuss with the Principal if there are volunteer opportunities in the building you wish to volunteer before completing the application and paying the $32 application fee.  This fee is non-refundable once the background check has been requested.  School Principals have the final authority over what happens in their school building, including the volunteer program and parent group activities. The contact information for each building principal is located on the Schools tab of the Middletown City School District website.

It is very important that the information you enter on the application matches your government issued identification card including your full legal name. It is important that you provide a valid email address so you can be notified as to the status of your application and for future communication. Current and prospective volunteers who will have unsupervised activities with students must have a satisfactory criminal background check prior to starting those activities and renewed every five years thereafter. All information collected on the application will remain confidential and not be shared outside the volunteer program.

As a volunteer for the Middletown City School District, I acknowledge that I am a role model for students and my actions need to reflect the professional conduct of an adult working with children.  I further understand that I must:

* Comply with all district policies, state and federal law

* Respect the confidential nature regarding students, families and staff of the school

*Immediately report to the principal or administrator any issues or concerns

* Make reasonable efforts to assure that each student is safe, protected from harassment and or discrimination

*  Understand that relationships developed with children at school should remain at school

* A volunteer must have completed a background check and be approved through the volunteer portal prior to having unsupervised interactions with any child.  The entire cost of the background check is the sole responsibility of the volunteer. 

By providing your signature, you are agreeing to abide by the rules, regulations and policies of the District Volunteer Program. One or more of the volunteer activities you have selected may require you to provide your United States social security number so we can perform a criminal background check. Your social security number will only be used for this purpose and we do not retain this information.
