Dear Volunteers,

All parents/guardians of students enrolled in the Shelby City School District are encouraged to take an active role in the education of their children. The Board of Education recognizes greater involvement on the part of parents and the community members in the education of children generally results in higher achievement scores, improved student behavior and reduced absenteeism. Certain programs and activities can be enhanced through the use of volunteers who have particular knowledge or skills that will be helpful to members of the professional staff responsible for the conduct of those programs and activities. Thank you for giving your valuable time to enhance our programs.

Please note, it is very important that the information you enter on the volunteer application matches your government issued identification card including your full legal name.  It is important that you provide a valid email address so you can be notified of the status of your application and for future communication.

Completing this form confirms that you have read the Shelby City School District Board Policy regarding school volunteers, and understand the following:

• I will abide by all Board policies and District guidelines while on duty as a volunteer (including, but not limited to, the volunteer’s obligation to keep confidential and not release or permit access to any and all student personally identifiable information to which he/she is exposed except as authorized by law).
• If I am volunteering in a position in which I will work unsupervised with children, I will be required to complete the background check process and the Shelby City School District will cover the cost associated with required background checks for volunteering completed through the Shelby City School District Raptor link when a background check is required.
• I will be covered under the District’s liability policy to the extent I am acting in good faith and within the scope of my role as a volunteer, but the District cannot provide any type of health insurance or coverage for personal injury, illness or damage incurred while serving as a volunteer.
• I am assuming the risks associated with my volunteer role and accept sole responsibility for any injury including, but not limited to, personal injury, disability, death, illness, or damage incurred while serving as a volunteer.
• I am not eligible for worker's compensation.
• If my background check shows a conviction or guilty plea for certain offenses, the District reserves the right to disapprove the volunteer/chaperone application.

I hereby release and agree to hold harmless and indemnify the Shelby City School District Board of Education, its members, employees, and agents, from any and all liability, arising from negligence or otherwise, as a result of my volunteer services, including, but not limited to, property damage and any mental or physical bodily injury.
