Covina-Valley Unified School District Volunteer Application

We appreciate your interest in volunteering for the District Volunteer Program. Our district offers a variety of activities you may choose from when volunteering your time. Simply complete and submit a volunteer application and upon approval you will receive instructions on next steps.

It is very important that the information you enter on the application matches your government issued identification card including your full legal name. It is important that you provide a valid email address so you can be notified as to the status of your application and for future communication. If you are unable to provide an email address, you use the district’s email address. All information collected on the application will remain confidential and not be shared outside the volunteer program.

Statement of Understanding

It is possible that as a visitor you may have more than occasional or infrequent contact with students. Under Penal Code 290.95 you are required to disclose to school officials if you are a registered sex offender. Your failure to disclose this fact could result in your arrest, prosecution, and likely fine and imprisonment. By placing your name below, you declare under penalty of perjury, that you are required, pursuant to Penal Code 290.95, to disclose to school officials that you are a registered sex offender, and that you have not suffered convictions for any violent or serious felony as defined in Sections 667.5 and 1192.7 of the Penal Code; or any sex offense as defined in Section 44010 of the Education Code; or any controlled substance offenses as defined in Section 44011 of the Education Code (EC 44010, 44011, or 44424); and there are no criminal charges against you. Furthermore, as provided under BP 1313 Community Relations Civility section, the Covina-Valley Unified School District Board of Education encourages parents/guardians and interested members of the community to visit the school sites while ensuring the safety of students and staff and avoid potential disruptions. This policy is not intended to deprive any person of his/her right to freedom of expression, but only maintain a safe working place for students and staff. The District seeks public cooperation with this endeavor.
