Sylvan Union School District Volunteer Application

We are extremely pleased that you are volunteering to assist our Sylvan Union School District students and staff. Your time, energy and commitment are greatly appreciated.

You must complete this online application in order to serve as a volunteer in our district. Some items you will need to provide include: current proof of tuberculosis (TB) clearance and a current copy of your driver's license. Depending on the level of volunteer you are applying for, you may be required to complete and provide the mandated reporter training certificate and fingerprinting. Simply complete and submit a volunteer application and sometime after submission you will receive instructions on next steps.

It is very important that the information you enter on the application matches your government issued identification card including your full legal name. It is important that you provide a valid email address so you can be notified as to the status of your application and for future communication. If you are unable to provide an email address, you can use your student's email address. All information collected on the application will remain confidential and not be shared outside the volunteer program.

If you have any questions about the application, please reach out to your school site. We look forward to working with you as a Sylvan Union School District volunteer.
