We appreciate your interest in Brookstone Schools! This application provides information to help us assure safety checks, since you will be working with students.

Potential Volunteers: We offer a variety of activities you may choose from to provide service. Simply check the boxes of all interests in the questionnaire section.

Potential Employees or Contractors: Please complete the necessary questions (red *). In the section of "interest", select Employee / Contractor.

ALL: It is very important that the information you enter on this application matches your government issued identification card including your full legal name. It is also important that you provide a valid email address, so you may be notified regarding the status of your application and for future communication.

Once you have completed and signed this application for submission, you will be notified of your initial approval. Final approval is contingent upon the completion of MinistrySafe video training, for those necessary. [An invitation will be sent to you via email. Upon completion of the required training, a certificate is issued.]
