University Schools Volunteer Application

Thank you for your interest in volunteering with University Schools. We greatly appreciate your commitment to our students, teachers, programs and schools. 

Please complete and submit this volunteer application by providing your full legal name, birth date and other information that matches your government-issued identification card (examples can be found on the Frequently Asked Questions) as well as accurate address, phone number and valid email address information so we can communicate with you about your application. 

If you do not have an email address, please enter volunteering@universityschools.com and University Schools Volunteer Coordinator will contact you via phone to update you on the status of your volunteer application. 

All information collected on the application will remain confidential and will be shared only with J.D. Palatine Company to conduct the background check and will not be shared outside of the volunteer program.

University Schools also requires that you provide your United States social security number, which will be used to conduct a criminal background check.  Your social security number will only be used for this purpose and we do not retain this information.  If you would like to volunteer with University Schools, but do not have a United States Social Security Number, please contact University Schools Volunteer Coordinator at volunteering@universityschools.com for application instructions.

I understand that University Schools is not liable for any accidental injury or illness suffered by me, or a third party, as a result of my volunteer activities. I also understand that University Schools will not be responsible for any claims made by third parties as a result of my volunteer activities.  I also understand that by submitting this volunteer application, I give University Schools permission to conduct a criminal background check.  Applicants with a United States Social Security Number are required to enter their information below, which will be used to conduct the criminal background check.  Applicants who do not have a Social Security number must contact the Volunteer Coordinator at volunteering@univrsityschools.com for alternate application instructions.

In connection with the volunteer applications, University Schools will procure a background investigative report and/or background report from JD Palatine Company as part of the process of considering your application for volunteer.  In the event that information from the report is utilized in declining an applicant’s volunteer status, University Schools will provide the applicant with a copy of the report at the applicant's request.

By signing below, you hereby authorize University Schools to obtain an investigative report about you in order to consider you as a volunteer.  Applicant further agrees to abide by the University Schools’ Volunteer Agreement, Confidentiality Agreement and Volunteer Code of Ethics.  This document can be found on the University Schools’ website, “Volunteering” page under the “Support Us” tab.

The information requested in the Volunteer application is being used strictly for a pre-volunteer background screening purposes in order to obtain accurate results.  Your Social Security number will only be used for this purpose, and we do not retain this information.
