Anacortes School District Volunteer Application

Dear prospective and returning volunteers,

Welcome, and thank you for your interest in volunteering for our schools!  We offer a wide variety of volunteer activities that support student learning, and we look forward to your participation as a partner in education.  Please read the following text carefully, so that you clearly understand each part of the application process.

BACKGROUND CHECKS
With the safety of students our highest priority, we conduct a national criminal history background check, including sex offender status, for all potential volunteers.  The information you enter on your application must match your government-issued identification card (passport, drivers license, or state ID), including your legal first and last name and social security number.  We do not retain your social security number once your background check is complete.  You also will need an active email address.  If you do not have one, you may wish to establish an account through Gmail, Yahoo, Outlook, or another free service.  Any information you provide will remain confidential and will not be shared with outside organizations.

Background checks can take a week or more to process.  You will receive notification via email at each step of the way, including a final notice when your application is approved or denied.  If your background check returns a finding, you will receive a copy of the report at the address you provide in the application.

INSTRUCTIONS
The volunteer application is compatible with PC desktops or laptops.  Apple users may find success with Firefox as a browser.  The application is not mobile-friendly.

To begin the process, please read through these instructions first, then click the "Next" button at the bottom right-hand corner of this screen.

On the PERSONAL INFORMATION page, please complete all required fields, then continue to scroll down to a series of three questions regarding other aspects of your volunteer profile such as military status and second language fluency.  You must complete these questions or enter "N/A" for any question that does not apply to your situation.  Once all questions have been answered, the application will allow you to move to the next screen.  NOTE:  Depending on your browser's window, the final questions may not be visible until you scroll down the page.

**APPLICANTS 17 & UNDER:  Please provide your SCHOOL email rather than a private or home email address.**

On the SCHOOL PREFERENCES and FUNCTIONS pages, you may choose as many options that seem suitable as you wish.  Preferences and Functions are potential areas of interest rather than a firm commitment to those choices.  Selecting these options does not commit you to these schools or activities, but it does enable schools to conduct outreach in those specific areas, and it also enables these categories during the Raptor login / name-badge process that volunteers must use each day they are on campus.   Some choices are specific to particular schools or represent affiliated organizations such as PTA / PTSA or booster clubs that may have additional membership requirements.  Programs such as AVID and tutoring or mentoring require an interview process and orientation.  Some activities may require prior experience or training, but most are open to all participants.  

On the DISCLAIMERS page, please read the text carefully, then enter your social security number and sign with your mouse or touchscreen.   Once you have entered all information, press "Next" to submit your application.

At this point, the Raptor system will generate an automatic message to the email address you entered, acknowledging submission of your form.  Upon approval, you will receive additional guidance on serving in our schools.  As mentioned above, some volunteer activities such as tutoring, mentoring, athletics, and AVID have additional requirements or are assigned through an interview process.  If you are interested in these areas of service, we will contact you with more information specific to those activities. 

IMPORTANT Volunteers must apply each school year.  Once approved, clearance extends through the current school year unless notified otherwise.  Volunteers serve at the discretion of the Anacortes School District, which retains the right to terminate a volunteer's status or activities at any time.

VOLUNTEER SIGNATURE & AUTHORIZATION FOR BACKGROUND CHECK
By providing my signature, I agree to abide in full by the rules and regulations of the Anacortes School District and its volunteer program.  If I am under the age of 18, my parent or guardian will be contacted to confirm my volunteer application.  The district's volunteer clearance process requires me to provide your United States social security number to enable a criminal background check. My social security number is used only for this purpose and is not retained by the Anacortes School District or its volunteer application system.  

VOLUNTEER CONFIDENTIALITY
In accordance with the Family Educational Rights and Privacy Act (FERPA), I agree to hold in confidence all information about individual students, including academic, emotional, social or family matters.  I will not share student information with anyone other than a teacher, principal or administrator.  I will promptly notify a teacher, principal or administrator should I have any concerns about a specific student.  

VOLUNTEER HOLD HARMLESS
I acknowledge that the Anacortes School District will make every attempt to ensure my safety while participating in the volunteer program but that there are certain inherent risks that may be unavoidable, resulting in bodily injury or property damage or loss to myself or others.  I further acknowledge that the district does not provide accidental medical insurance coverage for volunteer activities and that I assume all risks of injury or damage to my person or property.  I agree to hold and save harmless the Anacortes School District, its school board, employees, and assigns for any claims, suits or damages including but not limited to defense and indemnification which might result from my participation in the above described event.

VOLUNTEER CRIMINAL DISCLOSURE
For the safety of our children, the Anacortes School District requires all volunteers to participate in a national criminal background check each year.  Conviction or pending charges of any crime may preclude volunteer service in our schools, in particular conviction or pending charges of any crime against children or other persons, or of any crimes relating to the abuse or financial exploitation if the victim was a vulnerable adult; of crimes related to drugs; or of any dependency action, domestic relations proceedings, or disciplinary board final decision regarding the sexual abuse, assault or exploitation of any minor, or the physical abuse of any minor or developmentally disabled person.

I certify under penalty of perjury under the laws of the State of Washington that the information I have provided in this application is true and correct.  I authorize the Anacortes School District to make such investigations and inquiries as may be necessary to arrive at a volunteer assignment decision.  I understand that any offer of volunteer placement is contingent upon an acceptable outcome of the criminal history check as determined by the Anacortes School District.  I agree that the district may, at its discretion, preclude me from volunteer service if, among other reasons, I provide misleading or incomplete statements.  I understand that I am required to abide by all policies, procedures and regulations of the Anacortes School District.
