Fairview School District Volunteer Application

We appreciate your interest in volunteering with Fairview School District. Our district offers a variety of activities you may choose from when volunteering your time.

In order to volunteer, you will need to complete and submit a volunteer application and obtain volunteer clearances. Volunteer clearances include the ACT 34 PA Criminal History, the ACT 151 Child Abuse History and the ACT 114 FBI Fingerprint OR FSD Affidavit for most activities. Additional requirements may be necessary for some activities.

Instructions for how to obtain clearances are located on our district website (www.fairviewschools.org/volunteer). You may upload your clearances as part of your application or turn them in to the Volunteer Coordinator. Once your application and clearances are submitted, you will be notified of your approval as a volunteer.

It is very important that the information you enter on the application matches your government issued identification card including your full legal name. It is important that you provide a valid email address so you can be notified as to the status of your application and for future communication. If you are unable to provide an email address, you may use the district’s email address (volunteer@fairviewschools.org). All information collected on the application will remain confidential and not be shared outside the volunteer program.

3. Wearing a volunteer badge/identification at all times within buildings.

4. Using discretion when communicating about students to community members.

5. Maintaining up-to-date clearances and requirements.

6. Attesting that all information provided is true and accurate according to your knowledge.
