Thank you for your interest to volunteer with Eugene School District 4J.  Please complete the volunteer application in its entirety.  Upon approval, you will receive instructions on the next steps.

Information entered on the application must match your government-issued identification card including your full legal name.  A valid email address is required so you can be notified about the status of your application and for updates.  All information collected on the application is confidential and will not be shared outside of the volunteer program.

FCRA Disclosure and Authorization to Obtain a Consumer Report Eugene School District 4J Pursuant to the federal Fair Credit Reporting Act, I hereby authorize Eugene School District 4J and its designated agents, representatives, and consumer reporting agencies, such as OPENOnline, TLO, to conduct a comprehensive review of my background through a consumer report and/or an investigative consumer report to be furnished to Eugene School District 4J to use the information for employment, volunteering and contracting purposes. I understand that the scope of the consumer report and/or investigative consumer report may include, but is not limited to, the following areas: verification of Social Security number; current and previous residences; employment history, including all personnel files; education; references; credit history and reports; criminal history, including records from any criminal justice agency in any or all federal, state or county jurisdictions; birth records; motor vehicle records, including traffic citations and registration; and any other public records. I understand that information from the consumer report will not be used in violation of any applicable Federal or State equal employment opportunity law or regulation.

I authorize the complete release of the records or data pertaining to me that an individual, company, firm, corporation or public agency may have, including the information that I provided on the Criminal History Verification Form, to prepare the above-described consumer report. I hereby authorize and request any present or former employer, school, law enforcement agency, financial institution or other persons having personal knowledge of me to furnish Eugene School District 4J or its designated agents with any and all information in their possession regarding me in connection with an application of employment, volunteering or contracting services. I am authorizing that a photocopy of this authorization be accepted with the same authority as the original.

I understand that, pursuant to the federal Fair Credit Reporting Act, if any adverse action is to be taken based upon the consumer report, a copy of the report and a summary of the consumer's rights will be provided to me.
