By signing this application, you acknowledge that you have reviewed and agree to follow the rules, policies, and procedures of the Madison Elementary School District Volunteer Program.
You understand and agree to the following:
- If you are not the student's biological parent or legal guardian, you are required to complete an additional volunteer application and schedule a fingerprint appointment before volunteering.
- Volunteers serve under the supervision of school staff and may be dismissed from the volunteer program if their actions are not in the best interest of students or the District.
- Volunteers must maintain the confidentiality of student information and comply with all applicable privacy requirements.
- Volunteers may not discipline students, administer medication, provide transportation, or discuss student information with others.
- Volunteers are required by Arizona law to report suspected child abuse or neglect to appropriate school personnel.
- Volunteers must sign in and out through the school office each time they volunteer and wear their volunteer badge while on campus.
- Volunteers must maintain appropriate and professional communication and conduct with students at all times.
- Failure to follow District policies and volunteer expectations may result in the suspension or termination of volunteer privileges.
