Welcome to the Pewaukee School District Volunteer Program!

Thank you for your interest and enthusiasm in volunteering for the Pewaukee School District. We are thrilled to have you join our crew of amazing volunteers! Your involvement and support are invaluable in achieving our mission to open the door to each child’s future. 

There is a variety of volunteer opportunities for you to share your time, talents and energy that will make a positive impact in the lives of Pewaukee students and staff. Please take a few moments to complete this Pewaukee School District Volunteer Application. To get started, click the NEXT button below. Your responses will help us understand your interests, skills and availability, and enable us to match you with rewarding volunteer opportunities that align with your preferences and our current needs. It is important that the information you enter on this application matches your government-issued driver's license or official state photo identification including your full legal name and date of birth. Please also provide a valid email address that will be used to notify you of your application status and for future communication from Pewaukee School District. All information collected on the application remains confidential and is not shared outside the Pewaukee School District and volunteer program. 

As part of completing this application, you will be asked to consent to a criminal background check. This is a requirement and helps ensures the safety and security of student, staff, volunteers, visitors and guests. After initial approval, you will need to  present your current valid driver's license or state photo identification card for a one-time scan, which takes 1-2 minutes and can be done at any of the Pewaukee school offices or District office during school/business hours. This application must be completed and approved prior to you participating in any volunteer assignment. Approved volunteer applications are valid for three years from the approval date.

DISCLOSURE OF INTENT TO OBTAIN BACKGROUND REPORTS

For volunteer purposes, the Pewaukee School District, and/or JD Palatine, LLC. may obtain consumer reports or investigative consumer reports, in connection with your volunteer application or from time to time during a volunteering period in accordance with applicable law. Consumer reports include record checks conducted by consumer reporting agencies and may include driving records and/or criminal records. You may make a written request for a summary of consumer rights and a disclosure of the nature and scope of an investigation. A disclosure of the nature and scope of such investigation is provided below.

DISCLOSURE OF NATURE AND SCOPE OF INVESTIGATION FOR INVESTIGATIVE CONSUMER REPORT
In the event we request an investigative consumer report in connection with your volunteer application or during your volunteering period, a consumer reporting agency will prepare an investigative consumer report based on the investigation. The agency will also conduct a records check of driving and criminal records. Such a report will be obtained from JD Palatine, LLC. I agree I have received and/or reviewed a summary of my rights under the Fair Credit Reporting Act. 

AUTHORIZATION:

I authorize the Pewaukee School District and JD Palatine, LLC to obtain consumer reports and/or investigative consumer reports regarding me from time to time for volunteer purposes. Additionally, to the extent that JD Palatine, LLC obtains such reports, I consent to their release to the Pewaukee School District. In compliance with the Fair Credit Reporting Act, as amended by the Consumer Credit Reporting Reform Act of 2003, JD Palatine, LLC needs my authorization to obtain such a report.
