Azle Volunteer Application

Dear Volunteer,

Thank you for choosing to partner with Azle ISD!  A focus of the Azle ISD Strategic Plan is providing a World Class Education by partnering with family and community.

It is very important that the information you enter on the application matches your government issued identification card including your full legal name. It is important that you provide a valid email address so you can be notified as to the status of your application and for future communication. If you are unable to provide an email address, please use the district email address provided during the application process. All information collected on the application will remain confidential and will not be shared outside the volunteer program.

Volunteers are classified into two levels. 

Level I Volunteer

1. Requires a volunteer to always be in the presence of a district employee. 
2. Volunteers must sign-in at the campus and a Raptor Sex Offender Registry check will be conducted using a picture ID.
3. Function examples--cafeteria/office helper, lunch guest, guest speaker, class party, awards ceremony, supervised teacher helper.

Level II Volunteer

1. Permits a volunteer to support the school in a role that may be unsupervised and permit one-on-one interaction with students.
2. Volunteers must sign-in at the campus and a Raptor Sex Offender Registry check will be conducted using a picture ID.
3. A DPS Criminal History background check must be completed prior to Level II clearance being granted.
4. Function examples--event chaperone, one-on-one student assistance, field trips, tutor, field day, any other unsupervised activity

Azle ISD may obtain information about you for purposes of evaluating your application for a volunteer position from a consumer reporting agency, JD Palatine, 301 Grant Street, Suite 3400, Pittsburg, PA 15219 (877) 745-8525 ext. 1, by Azle ISD, or another outside agency. This procedure is established in order to create guidelines for checking criminal histories on volunteers that work on district campuses.  

Texas Education Code 22.083 states that the District may obtain criminal history record information that relates to a person the District intends to employ, a person that contracts with the district or a person who has indicated, in writing, an intention to serve as a volunteer with the District. Azle ISD policies GKG (Legal) and GKG (Local) establish a volunteer program and define a volunteer as "a person rendering services for or on behalf of the District on District premises or at a school-sponsored or school-related activity on or off school property who does not receive compensation in excess of reimbursement for expenses." 

By providing your signature, you are agreeing to abide by the rules and regulations of the Azle ISD's Volunteer Program. One or more of the volunteer activities you have selected may require you to provide your United States social security number so we can perform a criminal background check. Your social security number will only be used for this purpose, and we do not retain this information.
