District Volunteer and Visitor Application

We appreciate your interest in volunteering with and/or visiting Ball-Chatham Schools. Simply complete and submit an application which includes the background screening process, and you will receive instructions on next steps.

If you want to VOLUNTEER or CHAPERONE events or in the classroom as a helper, select ALL OPTIONS that match your needs.

If you want only to ATTEND a school event during the school day such as a music performance, please check the *VISITOR ONLY* option.

It is very important that the information you enter on the application matches your government issued identification card including your full legal name. It is important that you provide a valid email address so you can be notified as to the status of your application and for future communication. If you are unable to provide an email address, you will use the provided district’s email address. All information collected on the application will remain confidential and not be shared outside the volunteer program.
