LUESD Volunteer/Chaperone Application

Lemoore Union Elementary School District (LUESD) welcomes and encourages parents, guardians, grandparents, as well as other patrons of this district to become involved in neighboring schools by volunteering their spare time or by serving as a chaperone for school field trips.  We appreciate your interest in participating in the District Volunteer/Chaperone Program. Please complete the LUESD Volunteer/Chaperone Application and submit all information requested.  

NOTE:  Please be prepared to provide proof of Adult Tuberculosis (TB) Clearance Documentation at the time of application submission, by uploading the following file types (PDF, JPG, PNG).  Adult Tuberculosis (TB) Risk Assessment Certificate of Completion or a Tuberculosis (TB) Skin Test Clearance. The TB Clearance must be dated within the past sixty (60) days from the date of this application.  The 60 days may be waived, if a valid TB clearance copy is provided from another California school district within the last four (4) years, or for military or corrections personnel who may provide a TB clearance within the last 12 months.  Please contact the school site secretary for further guidance.     

All information submitted by applicants is confidential, and managed through Raptor Technologies' secure server and will not be shared outside the volunteer/chaperone program.  It is very important that the information you enter on the application matches your government issued identification card including your full legal name.  It is important that you provide a valid email address so you can be notified as to the status of your application and for future communication. If you are unable to provide an email address, you may use the district’s email address.  As part of standard school safety practices, all volunteers/chaperones and visitors to LUESD schools must provide a photo ID upon their initial visit to any District facility.  

Once you've completed the online application, you must visit the school site to have your government issued identification card scanned, in order for your application to be finalized. Upon approval of your application, you will receive instructions regarding next steps.

By signing below as a Volunteer, Chaperone or Student Observer, I hereby agree that I have read, understand and will abide by the rules and regulations of the District Volunteer Assistance Program (BP/AR 1240), Civility Policy (BP/AR 1265) and the following expectations and requirements: 

  • I am required to check-in at the office, register and receive my volunteer badge.  Badges are required to be returned to the office upon departure from the site. 
  • I shall be on time and keep my appointed volunteer, chaperone or observation days.
  • I shall be positive and report any problem(s) to the teacher.
  • I shall wear comfortable clothing, while keeping in mind the educational environment.  I will avoid “tank tops, spaghetti strap tops, halter tops, bare midriffs or bare chest, see-through outfits or off-the-shoulder blouses”.
  • While serving as a volunteer, chaperone or student observer, I shall not interrupt instructional time by usage of a personal cellular telephone or mobile communication device, except in an emergency.
  • As a parent (if applicable), I understand that I am in a supporting role as a classroom volunteer.  I will not ask questions about my child’s progress during classroom instructional time.
  • I understand that students are at different educational levels.  I may help the students by providing support and clarification, but I am not to do the work for them.
  • I am expected to maintain confidentiality regarding each student and family.  I will not discuss other student’s progress with other parents (including their own) and will direct any parent questions to the teacher.    
  • If I hear about or observe evidence of child abuse, I will report the information to the site principal or designee immediately. 
  • Education Code 35021 provides that a person who is required to register as a sex offender pursuant to Penal Code 290 shall not serve as a volunteer.  Registered sex offenders cannot serve as chaperones for field trips or other school activities.
  • I shall not be authorized to assign grades to students, and shall not be used to assist certificated staff in performing teaching or administrative responsibilities in place of regularly authorized classified employees who have been laid off. 
  • I shall act in accordance with district policies, regulations and school rules. The Superintendent or designee shall be responsible for investigating and resolving complaints regarding volunteers, chaperones and student observers.

Your District Volunteer/Chaperone application has been successfully submitted and will be reviewed in accordance with the district’s volunteer policy. In order for your application process to be finalized, you must visit the school site for which you wish to volunteer or chaperone, so your ID can be scanned. Your application will not be finalized until you have your ID scanned, regardless if the district scanned your ID the previous school year.  Once cleared to volunteer or chaperone, you will receive confirmation of your status via email.  If you have any questions, please contact the District Volunteer Program representative at your school site.
