PART I - ACKNOWLEDGEMENT AND AUTHORIZATION

By providing your signature, you are agreeing to abide by the rules and regulations of Trinity-Klein Lutheran Church and School.  One or more of the volunteer activities may require you to provide your United States social security number so we can perform a criminal background check. Your social security number will only be used for this purpose and we do not retain this information.

In connection with this application to serve at Trinity Klein Lutheran Church and School, I understand that a Background Check will be requested by Trinity for employment or volunteer purposes, whichever is applicable, from JDP, Inc. (in partnership with Raptor Technologies, Inc.). The Background Check may contain information about me relating to my criminal history, driving and/or motor vehicle records, social security number verification, verification of education or employment history.  Such reports may be obtained at any time after receipt of this Disclosure and Authorization and if I am hired or serve as a volunteer, whichever is applicable, throughout the course of my employment or volunteer service, as permitted by law and unless revoked by me in writing.

By signing this online application, I voluntarily and knowingly authorize Trinity Klein Lutheran Church and School to perform a background check.

PART II - CELL PHONE USE AGREEMENT (For Authorized Drivers)
Drivers who use cell phones in their vehicles have a higher risk of collision than drivers who don't, whether holding the phone or using a hands-free device.  Although Texas has no statewide law banning the use of cell phones while driving for all drivers, many local areas prohibit or limit the use of cell phones while driving. While children are in your care and supervision, we are asking all adult sponsors to also agree to not use their cell phones.  The children’s safety is our top priority.  We urge you to drive now and talk later. If you must make a phone call, pull over. Otherwise, wait until you reach your destination to use the phone.

By signing this online application, I agree to provide excellent supervision to the children entrusted to my care.  I will put away my cell phone and focus on my responsibility to the children.  I will under no circumstance, use my cell phone while driving Trinity Lutheran students to and from a school sponsored activity.
