School District of Menomonee Falls Volunteer Application

The School District of Menomonee Falls appreciates your desire to volunteer and values your time and commitment to our students, staff and families. If you would like more information on available volunteer opportunities, please contact the principal of the school or our Community Education and Rec Center.

During your time as a volunteer you may be given access to certain information about students and staff. The U.S. Congress has enacted the Family Educational Rights and Privacy Act that ensures our volunteers are mindful of confidentiality and expects that volunteers do not share or discuss any information or facts they may see or hear while volunteering.

To volunteer with the School District of Menomonee Falls, simply complete and submit the online volunteer application. Once your application is reviewed and approved, you will receive instructions on next steps. Please be advised that background checks can take up to 48-96 hours, during regular business hours Monday - Friday, to complete the automatic process.

It is very important that the information you enter on the application matches your government issued identification card including your full legal name. It is also important that you provide a valid email address so you can be notified on the status of your application and for future communication. If you are unable to provide an email address, please use the district’s email address as provided in the application process. All information collected on the application will remain confidential and will not be shared outside of the volunteer program.

By completing the application and providing my signature, I give permission for the School District of Menomonee Falls to conduct a background check on me, which may include a review of sex offender registries, child abuse, and criminal history records. I understand that if appointed my position is conditional upon the District receiving no inappropriate information on my background. I hereby release and hold harmless from liability the School District of Menomonee Falls, and its predecessors, successors,assigns, agents, directors, officers, employees, representatives, and all other persons acting by, through, under or in concert with any of them from any and all claims, known or unknown. I also understand that regardless of previous appointments, the School District of Menomonee Falls is not obligated to appoint me to a volunteer position. If appointed, I understand that I am subject to suspension by the District and removal by the Board of Education for violation of District policies or principles.

Your social security number will only be used for this purpose and we do not retain this information.The District reserves the right to complete an updated background check at any time and will alter opportunities available to a volunteer should it be deemed necessary to ensure our students and staff remain in a safe an healthy environment.

- I understand I am expected to maintain strict confidentiality about students and staff. This includes health issues, academic performance and behavior, school problems, and student records in any form. The access to records by a volunteer shall be limited to legitimate educational interests particular to the duty performed and under the supervision of a District employee acting as the volunteer's supervisor. (Student Records Policy #347)

- I understand I am to refrain from discussing school practices, individual students and personnel, or personal opinions regarding all children, adults, and situations observed while volunteering unless there is a legitimate educational interest to protect the safety interests of individuals in the school. If you have a specific concern, discuss it with the principal.

- I understand the School District of Menomonee Falls does not tolerate any discriminating or harassing behavior from students, staff, volunteers, or other individuals interacting with students. (Non-Discrimination Policy #411)

- I understand I am required to sign in and wear a designated badge when volunteering in the School District of Menomonee Falls.

- I understand I may be removed as a volunteer at any time for failure to adhere to District Policies and guidelines.
