North Hills School District Volunteer Application

Thank you for your interest in volunteering for the North Hills School District. This form should be completed after volunteer clearances have been submitted to the school building where you are intending to volunteer.

You will be notified via automatic email of your approval to volunteer.

When completing this application, please note:

Dates entered on this form must match the exact dates indicated on the official clearances
You will be asked to select a volunteer type. Descriptions are provided below.

Assistive Volunteer

Works under direct supervision and direction of an adult employed by the district
Does not provide direct services to students or have unsupervised contact with students

Examples of Assistive Volunteers include but are not limited to: volunteers who assist with school activities, holiday parties, field trips, picture day, field day, PTA/PTO/PTSO clubs, PTA/PTO/PTSO School volunteers (helping hands, lunch/recess, office, etc.)

Assistive Volunteers must have superintendent or designee approval prior to serving as an Assistive Volunteer in the District.

Program Volunteer - High School/Middle School Only (Applications for “Program Volunteer” require school board approval and may have an additional wait time for approval.)

Works under the general direction and supervision of an adult employed by the district
Provides direct services to students or may possibly have unsupervised contact with students

Examples include but are not limited to volunteers who assist the coaching staff of an athletic team, volunteer athletic trainers or equipment managers, chaperones for overnight student trips and choreographers, musicians and other individuals who provide assistance to students in the marching band or school musical and club/activity sponsors.

All information collected on this application will remain confidential and not be shared outside the volunteer program.
