We Appreciate your interest in volunteering for the Fort Worth ISD.

Please have your photo identification card with you and ready to be uploaded into this application. For safety, the application cannot be saved and, for this reason, you must complete and submit your application with your photo identification at the same time.

Following are the valid types of identifications for background checks:
     - Current driver's license issued by any of the United States or any of its territories.
     - Current Texas photo identification issued by the state
     - Current U.S. Military identification
     - Current U.S. Passport
     - Current Student/Work Visa
     - Resident card issued by the federal government of the United States
     - Any photo identification issued by a U.S. Government agency

The information you enter must match your government-issued identification card, including your full legal name. All information collected on the application will remain confidential and not be shared outside the volunteer program.

Terms and Conditions

• I, the undersigned, grant the Fort Worth Independent School District (FWISD) permission to obtain copies of any information about any criminal history record maintained by any law enforcement agency and use said information to evaluate my application for volunteering.  Further, I authorize consent without reservation, any party or agency contacted by FWISD to furnish the above information.
• By submitting this application, I verify that the facts set forth in it are true, complete, and given voluntarily.  I understand that if I am accepted as a volunteer, any false statements, omissions, or other misrepresentations made by me on this application may result in my immediate dismissal.
• I understand that this is an application, not a commitment or promise of a volunteer position.
• I understand that I will be required to complete an orientation or training session, or both before I begin volunteering.
• I understand that confidentiality is a crucial aspect of my volunteer assignment.  Aside from FWISD staff, I agree to hold any observations or information regarding students, families, or staff in the strictest confidence.
• In the event of a medical emergency, I hereby consent to the necessary and proper treatment, surgery, or anesthesia by a licensed physician or health care professional.  
• I understand that FWISD does not provide insurance coverage for losses, sicknesses, or injuries that may occur while participating in the program.  I am responsible for providing my insurance coverage and any applicable medical expenses incurred.
• I hereby give FWISD, its licensees, and legal representatives the right to use my name (or any fictional name, picture, portrait, photograph, videotape, or audiotape of me in any lawful manner.
• I understand that I will NOT be paid or otherwise compensated for my services as a volunteer.  No contract or agreement of employment is created by any written or oral representations made in connection with the FWISD Volunteer Program or in connection with any other program of FWISD.
• I am aware and agree that as a volunteer, I may be dismissed from the program at any time, for any reason, or FWISD may discontinue the program at any time.
• I agree to read the FWISD Volunteer Information Guide, and I agree to abide by any and all the FWISD policies, rules, and regulations.
• I AGREE TO INDEMNIFY AND RELEASE THE FORT WORTH INDEPENDENT SCHOOL DISTRICT, ITS REPRESENTATIVES, AND EMPLOYEES FROM ANY AND ALL LIABILITY RELATED TO THE VOLUNTEER ACTIVITIES.  as a participant in the FWISD volunteer program, I understand my participation may include off-site activities, including field trips, outings, and excursions at locations other than the district administration offices or its school facilities.  IN CONSIDERATION FOR ALLOWING ME TO PARTICIPATE IN THE FWISD PROGRAM, I DO HEREBY AGREE TO RELEASE, DISCHARGE, INDEMNIFY, AND HOLD HARMLESS THE FORT WORTH INDEPENDENT SCHOOL DISTRICT, ITS AGENTS, REPRESENTATIVES, VOLUNTEERS, AND EMPLOYEES, FROM AND AGAINST ANY AND ALL CLAIMS, DEMANDS, CAUSES OF ACTION, LIABILITIES, DAMAGES, PERSONAL INJURIES, INCLUDING DEATH, PROPERTY DAMAGE, ECONOMIC LOSSES, OR LOSS OF ANY NATURE OR CAUSE, INCLUDING COSTS AND ATTORNEY'S FEES IN CONNECTION WITH OR ARISING FROM ANY PARTICIPATION IN THE FWISD VOLUNTEER PROGRAM, INCLUDING DAMAGES OR INJURIES SUSTAINED AS A RESULT OF THE NEGLIGENCE OF THE FWISD OR ITS EMPLOYEES OR PARTICIPANTS IN THE PROGRAM.

DPS Computerized Criminal History (CCH) Verification

• I have been notified that a Computerized Criminal History (CCH) verification check will be performed by accessing the Texas Department of Public Safety Secure Website and will be based on the name and date of birth identifiers I supply.
• Because the name-based information is not an exact search and only fingerprint record searches represent true identification of criminal history, the organization conducting the criminal history check for background screening cannot discuss any criminal history record information obtained using the name and date of the birth method.  Therefore, the agency may request that I have a fingerprint search performed to clear any misidentification based on the result of the name and date of birth search.
• For the fingerprinting process, I will be required to submit a full and complete set of my fingerprints for analysis through the Texas Department of Public Safety AFIS (Automated Fingerprint Identification System).  I have been made aware that to complete this process, I must make an appointment with L1 Enrollment Services, submit a full and complete set of my fingerprints, request a copy be sent to the agency listed below, and pay a fee of $24.95 to the fingerprinting services company, L1 Enrollment Services.
• Once this process is completed and the agency receives the data from DPS, the information on my fingerprint criminal history record may be discussed with me.
• (This copy must remain on file by your agency. Required of future DPS Audits)
