1. Acknowledgment of Risk I, the undersigned, understand and acknowledge that participating in volunteer activities with Miami Valley Christian Academy may involve inherent risks, including, but not limited to, physical injury, illness, and property damage. I acknowledge that I am voluntarily participating in these activities and am aware of the risks involved.
2. Assumption of Risk I assume full responsibility for all risks of bodily injury, death, or property damage that may occur while participating in volunteer activities at or for Miami Valley Christian Academy.
3. Waiver and Release of Liability In consideration of being permitted to volunteer for [School Name], I hereby release, waive, discharge, and hold harmless Miami Valley Christian Academy, its employees, administrators, agents, and affiliates from all liability, claims, demands, or causes of action that may arise from my participation in volunteer activities, whether caused by negligence or otherwise.
4. Indemnification I agree to indemnify and hold harmless Miami Valley Christian Academy, its employees, agents, and representatives from all claims, actions, damages, and expenses (including reasonable attorney’s fees) arising from my volunteer activities.
5. Medical Treatment In the event of an injury or illness, I give my consent to Miami Valley Christian Academy to secure medical treatment deemed necessary, understanding that any costs related to medical treatment will be my responsibility. I acknowledge that Miami Valley Christian Academy is not obligated to provide medical insurance coverage for volunteers.
6. Compliance with School Policies I agree to abide by all Miami Valley Christian Academy policies, including safety and health protocols, and to follow the instructions provided by [School Name] staff and administrators during my volunteer service.
7. Media Release I understand that photographs or videos may be taken during volunteer activities. I grant that Miami Valley Christian Academy has permission to use my likeness in promotional materials, including print, online, and social media, without compensation.
8. Confidentiality I understand and agree to respect the confidentiality of all students, families, and school-related matters. I will not disclose personal or sensitive information gained through my volunteer work at Miami Christian Academy.
