District Volunteer Application

Dear OVSD Volunteer,

Thank you for your interest in becoming a volunteer. You are an integral part of Ocean View School District.  Your participation enhances the instructional program and motivates and encourages students. Before starting the application process, we recommend having your
 TB clearance/risk assessment ready and please be sure to review the Volunteer Handbook found on the District's Volunteer homepage. A volunteer is a parent, community member or other adult who assists at a school site or program on a regular or semi-regular basis before, during, or after school hours.  There are two types of volunteers.  Please select:

Tier I Volunteers: May perform service on the school campus more than five hours a week; chaperone on field trips; attend or chaperone overnight school-sponsored trips; and be a volunteer coach, assistant coach, or co-curricular assistant.  As a Tier I volunteer, you will need to complete this form and be fingerprinted (Note: There is a cost of approximately $48-$54 for fingerprint clearance); be checked against the Megan’s Law registry; and have a current Tuberculosis Risk Assessment/Examination (required every 4 years).


Tier II Volunteers: May perform service on the school campus for five hours or less per week; and includes parents who provide service participation hours as outlined by State Preschool requirements. As a Tier II volunteer, you will need to complete this form and be checked against the Megan’s Law registry every year, and have a current Tuberculosis Risk Assessment/ Examination (required every 4 years).

If Renewing Your Application: For Tier 1 volunteers, you do not need to repeat fingerprint clearance. Once you complete it for the District, you do not need to resubmit fingerprint clearance. For Tier 1 and Tier 2 renewing volunteers, please attach your updated TB clearance document.

Note: Immunizations are required for preschool volunteers; please contact the school office for additional information.

I am aware that during the course of my volunteering, confidential information may be made available to me. I understand that it is critical that this information not be disseminated within or outside the school community.

I understand that my volunteer services are terminable at will, either by myself or Ocean View School District, regardless of the length of my volunteering.

If you selected a Tier II function, you will be notified as to your volunteer status in the near future.

Note: Your approval at ‘All sites’ will not cleared at the preschools until you submit proof of vaccinations.
