The information you enter on this application must match your government-issued identification cards, including your full legal name, date of birth, and social security number. Any errors in this information can slow down the processing of your application or cause it to be denied. It is also important that you provide a valid email address so you can be notified as to the status of your application and for future communication. An image of your driver's license or state-issued photo ID is required to submit this application. Applicants under the age of 18 who wish to volunteer may upload an image of their current school photo ID badge, learner's permit, or state identification card. There is a section in the application to UPLOAD this image. All information collected on the application will remain confidential and shall not be shared outside the volunteer program.
