District Volunteer Application

We are excited to have you join us in Champaign Unit 4 Schools! There are a few things we need from you to ensure your experience is as enjoyable as possible and to protect our students' safety and welfare. The following information must be completed in order to be considered for a volunteer experience in Champaign Unit 4 Schools. Feel free to use the translation feature of your browser to fill out the application if needed.

1. Read the Volunteer Guidelines and Ethics document linked below. (If you click the link below, you will have to use the "back" arrow on your browser to return to this page.) You will be asked to sign a disclaimer page confirming that you have read the Volunteer Guidelines and Ethics as you proceed with this application.

2. You will complete a Volunteer Application in the next step. It is important that the information you enter on the application matches your government issued identification card, including your full legal name. It is also important that you provide a valid email address so you can be notified of the status of your application. (If you do not have an email, check with your school volunteer coordinator.) If you are a Student Observer, make sure you answer the questions at the bottom of the form about your placement. If you are a CU One to One Mentor, you will also need to complete a mentor application, which is available from your school mentor coordinator or at www.cu1to1.org.

3. You must review and complete the Background Check Disclosure and Authorization Form and Summary of Rights. All volunteers must successfully complete a criminal background check prior to beginning their volunteer experience. For most of you, a name-based check is conducted. You will be sent a link via email to complete your background check once your application has been accepted. (If a fingerprint-based check is needed, we will contact you to explain this process once we receive your application.)
