Thank you for your interest in volunteering with Greeley-Evans School District 6. We greatly appreciate your commitment to our students, teachers, programs and schools. Please complete this volunteer application by providing your full legal name, birth date and other information that matches your government-issued identification card as well as accurate address, phone number and valid email address information so we can communicate with you about your application, available volunteer opportunities and volunteer appreciation efforts. If you do not have an email address, please enter getinvolved@greeleyschools.org and a District 6 employee will contact you via phone to update you on the status of your volunteer application. All information collected on the application will remain confidential and will be shared only with J.D. Palatine to conduct the background check, and with District 6 volunteer program coordinators for communication purposes only.

District 6 also requires that you provide your United States social security number, which will be used to conduct a criminal background check. Your social security number will only be used for this purpose and we do not retain this information. I understand that Greeley-Evans Weld County School District 6 is not liable for any accidental injury or illness suffered by me, or a third party as a result of my volunteer activities. I also understand that Greeley-Evans Weld County School District 6 will not be responsible for any claims made by third parties as a result of my volunteer activities. I also understand that by submitting this volunteer application, I give Greeley-Evans Weld County School District 6 permission to conduct a criminal background check (CBC) in accordance with its Board of Education's Policy KIA - "Volunteers in Schools." Applicants with a United States Social Security Number are required to enter their information below, which will be used to conduct the criminal background check. In connection with all volunteer applications, District 6 will procure a Background Investigative Report and/or Background Report from JD Palatine as part of the process of considering your application to volunteer. In the event that information from the report is utilized in declining an applicant’s volunteer status, District 6 will provide you with a copy of the consumer report.
